Yes.

To be considered for employment, Case Managers must have a bachelor’s or master’s degree in a relevant major such as social work, human services, psychology, counseling, etc. 


If your resume can support two years of relevant work experience in case management / care planning, in addition to a master‘s or bachelor’s degree in a non-relevant major, you may apply for consideration.

Resume's for case management positions are reviewed and approved by an outside vendor. Resume’s that do not meet the degree requirements AND related work experience will not be considered. 


Applicants must have no less than two years experience in the following areas: Assessment, Care Planning, Service Counseling, Monitoring and Evaluation, Case Documentation, Privacy / HIPAA. Certified Case Manager certification or social work licensure is not required at this. Applicants with additional certification and licensure are encouraged to apply. 




We provide a fair and competitive pay structure to case managers based on experience and qualifications. Your exact rate will be confirmed with the submission of your official college transcript.

Case Managers are paid per case and NOT per hour. Clarification will be shared during the pre-screening interview.

Our office team members are employed in a part time capacity, working as business needs require. Office team members observe all local, state, and federal holidays without compensation.

The company utilizes a payroll processor with direct deposit available. The company follow a bi-weekly pay schedule. Taxes and unemployment insurance is deducted from your wages.

Training is provided without compensation. Once on-board, additional mandatory trainings and meeting are not compensated.

Once you have met the qualifications for employment through a knowledge based assessment, Case Managers will have 30 days to submit their SLED background report, PPD / TB skin Test, and Official College Transcript. Financial obligations to obtain these items are not funded by the company. 



Yes.

This is a work from home / mobile position. As a Case Manager, you will be required to use your personal vehicle to make in home visits with individuals who are disabled, aging, or terminally ill. 


Travel is expected approximately 25% to 35% of the time. The company does not provide car maintenance / repairs, gas/mileage reimbursements, or meal reimbursements.

A tablet or laptop is preferred when making in home visits. Case Managers are responsible for all relevant materials and devices required to perform job assignment. Items included but not limited to: Cell Phone, Tablets, Laptops, Computers, Printers, Ink, Paper, Notebooks, Internet/Data plans, and other office supplies or devices. The company will not reimburse  for the purchase of the items necessary to perform job assignments. 



As a Case Manager, considering the availability of the client, you will set your own schedule using the mandatory guidelines: Client visitation allowed between 7am - 7pm; Monday - Friday. Client Phone Calls made BEFORE 7pm, Monday - Friday.

Case Managers are required to maintain a flexible schedule between 8:30am - 5pm, Monday - Friday, to verbally or electronically communicate and respond to providers, social agencies, community partners, and state staff. Scheduled Case Manager meetings and trainings are mandatory to attend via in person or electronically. Exceptions evaluated on a case by case basis.

Weekend work is not required or allowed. If the client prefers a visit on the weekend, the case manager will request approval prior to scheduling weekend visitation with a client.




All Case Manages and Office Staff must follow HIPAA and Privacy guidelines when utilizing social media for networking and marketing purposes. Case managers will maintain and respect the Cleint privacy at all time. This includes but is not limited to, recording inside of a clients home, showing a client face on camera, recording conversations, revealing medical records and other personal info. 


When representing Grow In Grace Care Management online, at health & career fairs, and social networking setting; Employees must maintain professionalism. Direct unknown answers or complex questions to our website, office staff, or email. 


Negative comments made public pertaining to clients, their families, employees, or the company, is strictly prohibited. Termination will be considered and may result in legal action. Employees are able to report any client, family, or company grievances privately and respectfully.
