Yes.

To be considered for employment with Grow in Grace Care Management, applicants must hold a bachelor’s or master’s degree in a relevant field such as Social Work, Human Services, Psychology, Counseling, or a related discipline.


Applicants should also have at least two years of relevant work experience in case management, care coordination, or service planning. 


If you hold a degree in a non-related field but have significant professional experience in case management or human services, you may still be considered for employment. 


All resumes for case management positions are reviewed and approved by an external credentialing vendor to ensure compliance with state program requirements. Applications that do not meed the education and experience criteria will not be considered for employment. 


Applicants must have at least two years of professional experience in one or more of the following areas: 

  • Case management of care coordination
  • Social services or Human services
  • Counseling or mental health support
  • Healthcare or long-term settings
  • Client advocacy or resource coordination


Preferred candidates will demonstrate strong organizational skills, the ability to manage multiple cases independently, and a commitment to providing compassionate, client-centered care. 


Additional qualifications that strengthen an application include: 

  • Knowledge of South Carolina's Community Long Term Care (CLTC) or Healthy Connections programs
  • Familiarity with Medicaid waiver services and documentation
  • Proficiency in electronic case management systems and data entry
  • Reliable transportation and willings to travel for home visits

Grow In Grace Care Management reserves the right to withdraw or modify and offer of employment if an applicant fails to meet these requirements, provide false or incomplete information during the hiring process. 


Grow In Grace Care Management provides a fair and competitive pay structure to case managers based on experience and qualifications. Exact compensation rates are confirmed after submission and verification of your official college transcript.  


Case Managers are paid per case, not per hour. Compensation details and expectations will be discussed during the pre-screening interview.


Office team members are typically employed in a part time capacity, working as business needs require. Office staff observe all local, state, and federal holidays; however, these holidays are unpaid.

The company uses a secure payroll processor with direct deposit and follows a bi-weekly pay schedule. All applicable taxes and unemployment insurances are automatically deducted from wages.  


Training: 

Initial orientation and training are provided without compensation. Once on-boarded, participation in additional mandatory meetings or trainings are non-compensated.  



Employment Requirements: 

Upon meeting the qualifications for employment through a knowledge-based assessment, Case Managers will have 30 days to submit: 

  • SLED background report
  • PPD/TB skin test
  • Official college transcript.

Yes - with field-based responsibilities.

Case Managers with Grow in Grace Care Management primarily work remotely to complete documentation, case coordination, and communication with clients, providers and community partners. 


However, this position also requires in-person field visits to client homes, provider agencies, or community settings as needed to ensure compliance, perform assessments, and maintain quality assurance. 


Employees must have: 

  • Reliable transportation
  • Access to a secure and quite workspace suitable to handling confidential information 
  • A computer or laptop with internet access and proficiency in basic technology systems used for documentation. 
  • A tablet or small laptop with data or wifi connection to use for client visitation. 

All case managers are expected to manage their time responsibly, maintain confidentiality in accordance with HIPAA guidelines, and represent Grow In Grace Care Management with professionalism in both remote and in-person settings.


What Equipment or Supplies Are Required? 

Case Managers are responsible for maintaining the tools and technology necessary to perform their job duties efficiently and securely. Grow in Grace Care Management does not supply company owned equipment. 


All Case Managers must have: 

  • A reliable computer, laptop, or tablet capable of running secure web-based software. 
  • High-speed internet access for virtual documentation and communication. 
  • A mobile phone for contact with clients and agency partners. 
  • Access to a printer and scanner for required forms and documentation.
  • A secure and confidential workspace that complies with HIPAA privacy standards. 

Additional items such as milage logs, notebooks, ink, printer paper, postage, envelopes, pens, tablets, and other office or administrative items, are considered personal supplies and are the responsibility of each Case Manager. 


All client-related documentation must be completed and stored using approved systems provided by Grow in Grace Care Management. Any use of personal storage platforms (e.g., Google Drive, personal email, USB drives) for client date is strictly prohibited. 




Work Schedule: 

Case Managers with Grow in Grace Care Management work a flexible scheduled based on client needs, documentation deadlines, and state compliance requirements. While hours are not set, Case Managers are expected to: 

  • Maintain regular communication with clients and supervisors during standing business hours (Monday - Friday, 8:30am - 4:30pm) 
  • Respond to calls, messages, or documentation request in a timely manner. 
  • Meet all required case documentation, visits, and follow-up deadlines as assigned. 

While flexibility is provided, reliability and accountability are essential to ensure continuation of care for our clients and to maintain a positive, team-centered work environment. 


Service Areas: 

Grow in Grace Care Management currently serves client throughout South Carolina.

Case assignments are distributed based on client location, case volume, and team capacity. Assignments are made with consideration to each Case Manager's workload, availability, and service area coverage. Request for reassignment or additional cases must be submitted to the administrative team for review. 


Social Media Policy:  

As Representatives of Grow in Grace Care Management, all employees are expected to maintain professional conduct on social media and other online platforms. To protect our clients, agencies, and partners: 

  • Do not post or share client information, names, or images under any circumstances. 
  • Avoid engaging in public discussion, post, or comments that may reflect negatively on the agency, its clients, or its employees. 
  • Personal social media activity should not interfere with work responsibility or the agency's reputation. 
  • Any violation of confidentiality, HIPAA standards, or professional boundaries online may result in immediate termination.

We are currently hiring for Case Managers in SOUTH CAROLINA. In Home Visitation is required with an estimate of 25% to 35% of travel time each month. This is a work from home / mobile position where you will set your own scheduled according to set guidelines. If resume is approved and you make a passing score on the knowledge assessment, you will be require to submit a SLED background check, PPD / Skin Test, and official college transcript, within 30 days of receiving your passing score.
