Grow In Grace Is a Care Management company. Our skilled case managers evaluate an individual needs though assessments and determine eligibility for services.
Grow In Grace Care Managers utilize a person centered care plan that outline strengths, preferences, and desired goals. Through service coordination, your Grow In Grace Care Manager will connect clients and/or their family to home and community based service waivers, Medicaid and Non-Medicaid resources.
Our Care Managers will evaluate, advocate, and monitor our clients health and welfare and the Integration of the care plan.
Care Managers provide education + support to help clients and families make informed decisions regarding their care. Care managers help to identify a range or resources to include community support, social services, and education on health care needs.
When necessary, Care Managers will submit referrals and provide crisis management as needed.

Grow In Grace Care Managers are not licensed medical professional. Our Care Managers cannot write prescriptions, diagnose or provide medical or mental health treatment.
Without your consent, Grow In Grace Care Managers are unable to speak to your doctors, nurses, or insurance provider on your behalf.
With your consent, Care Managers may be able to arrange transportation and schedule medical and mental appointments when necessary. Care Managers are able to coordinate care with hospice providers, home health services, and other mental or medical professionals.
Grow In Grace Care Managers are not medically authorized to order hospital beds, wheelchairs, walkers, canes, power scooters, oxygen tanks, sleep apnea equipment, shower chairs, raised toilet seats, and other durable medical equipment. Such request must be handled by a medical professional such as a doctor or nurse practitioner.
Grow in Grace Care Managers, manage home and community based waivers for members of the South Carolina Healthy Connections program. With your consent and participation, Care Managers can submit a referral on your behalf to connect you with a Long Term Care representative. For members with other insurance policies, at no expense to you, our Care Managers are able to review your policy documents to determine your program benefits and/or connect you to private pay resources.
Utilizing your South Carolina Healthy Connections Benefits, Care Managers are able to secure home delivered meals, nutritional supplements, participation in adult day health centers, incontinent supplies, personal care / companion services, personal emergency response systems, ramps, bath safety equipment, pest control, and other covered waivers. If you need help enrolling into a healthy connection insurance plan or enrolling in to the waiver program, a Grow In Grace Care Manager can help you get started.
Grow in Grace Care Managers do not directly provide financial assistance, utility/bill payments, non emergency medical transportation, rental or housing assistance. Care Managers are able to collaborate with our clients, families, and partners to identify and connect to resources that may be suitable to meet the need.
