Grow In Grace Is a Care Management provides comprehensive case management and care coordination services for individuals and families across South Carolina. Our skilled case managers complete individualized assessment, determine eligibility for services, and develop person-centered care plans that outlines each client's strength, preferences and goals.  


Through ongoing service coordination, we connect clients and families to Home and Community-Based Services (HCBS) Waivers, including Medicaid and Non Medicaid programs, to support independence and quality of life. 


Our Care Managers:

  • Evaluate needs, advocate for clients, and monitor overall health and well being.  
  • Collaborate with healthcare providers and community partners to  ensure consistent, high-quality care. 
  • Provide education and guidance to help families make informed  decisions about available services. 
  • Assist with referrals, community support, social services, and crisis management when needed. 

At Grow In Grace, our goal is to walk beside you-helping you access the right resources, care, and support at every stage of your journey.

Grow In Grace Care Managers are not licensed medical professional and therefore cannot write prescriptions, diagnose conditions, or provide medical or mental health treatment. 


Without your written consent, our Care Managers are unable to communicate directly with your doctors, nurses, or insurance providers on your behalf. 


With your written consent, Care Managers may

  • Arrange transportation to and from medical appointments. 
  • Help schedule follow-up visits or therapy sessions. 
  • Coordinate care with hospice providers, home health agencies, or other healthcare professionals to ensure community support.  

Please note that Grow in Grace Care Managers are not authorized to order medical equipment such as hospital beds, wheelchairs, walkers, canes, power scooters, oxygen tanks, or shower chairs. These request must be completed by a licensed medical provider such as a physician or nurse practitioner. 


At Grow in Grace Care Management, our role is to advocate, coordinate, and support - ensuring you have access to the right professionals and resources to meet your needs safely and effectively. 


Grow in Grace Care Management partners with individuals and families who qualify for Home and Community-Based Services (HCBS) under the South Carolina Healthy Connections program. 


With your consent and participation, our Care Managers can submit referrals on your behalf and connect you with a Community Long Term Care (CLTC) representative to determine program eligibility and available benefits. 


For members with private pay insurance or other coverage, our team can review policy documents and help identify private-pay options or alternative community resources that fit your needs and budget. 


Our goal is to ensure that every client-regardless of coverage-has access to the support, services, and guidance needed to live safely and independently in their home environment. 


Through your South Carolina Healthy Connections benefits, Grow in Grace Care Managers can help you access a wide range of home and community based services designed to support safety, independence and quality of life. 


Depending on your eligibility and program type, services may include: 

  • Home delivered meals and nutritional supplements
  • Participation in adult day health programs
  • Incontinent and personal care supplies
  • Personal care or Companion services
  • Personal emergency response systems (PERS) 
  • Home modifications such as ramps or bath safety equipment
  • Pest control and other waiver services. 


If you need help enrolling in a Healthy Connections insurance plan or applying for the Home and Community-Based Waiver (HCBS) program, a Grow in Grace Care Manager can guide you through the process and connect you to the right resources. 


Grow in Grace Care Managers do not directly provide financial assistance, utility or bill payments, non emergency medical transportation, or housing assistance. 


However, our Care Managers collaborate closely with clients, families, and community partners to identify and connect you with local resources that may meet your specific needs. This can include programs that offer rental support, energy assistance, or other forms of temporary aid.
